1. Fill-up Inspection and Estimation Form for new water connection at the METRO VIGAN WATER DISTRICT (MVWD) office at P. Burgos St., Solid West, Vigan City.
2. MVWD personnel visit the location where the water connection will be installed, conducts an inspection and prepares a report on the estimation of materials to be used. A duplicate copy of the Inspection and Estimation Form will be furnished
to the applicant.
3. After Inspection, the applicant visits the District's office to fill up an Application for Membership and Contract for Service Connection. The applicant brings the duplicate copy of the Inspection and Estimation Form with the following requirements:
- 2 pcs. 1X1 picture (latest)
- Brgy. Certificate of Residency
- 1 Valid ID
- PostalID
- Driver's License
- SSSID
- Passport
- CompanyID
- Senior Citizen ID
- Phil. Health ID
- TIN ID
- Voter's ID
- GSIS / E-CARD ID.
4. The applicant pays the necessary fees and materials to be used for the water connection at the Cashier's office.
5. The water connection will be installed two to five (2-5) days upon payment of the necessary fees and upon completion of excavation (by the applicant) for the installation.
